Mixed Urinary Incontinence – When Stress and Urgency Overlap
Mixed urinary incontinence occurs when a person experiences both stress and urgency-related leakage.
This presentation is common, but often confusing, and requires a tailored, individual approach.
What Is Mixed Urinary Incontinence?
Mixed urinary incontinence includes:
- Leakage with coughing, sneezing or exercise (stress component)
- Leakage associated with urgency (urge component)
Symptoms may fluctuate, with one component more dominant at different times.
Why Mixed Incontinence Happens
Mixed incontinence reflects multiple contributing factors, such as:
- Pelvic floor weakness or poor support
- Pelvic floor overactivity or guarding
- Bladder sensitivity
- Nervous system dysregulation
- Constipation
Because more than one mechanism is involved, management must be prioritised carefully.
Assessment Is Essential
Assessment helps identify:
- Which component is most problematic
- Whether strengthening or relaxation should come first
- How bladder habits and load influence symptoms
Treating only one aspect often leads to incomplete improvement
Management Strategies
Management may involve:
- Pelvic floor muscle training (where appropriate)
- Bladder retraining strategies
- Pelvic floor relaxation and coordination work
- Behavioural and lifestyle modifications
Sequencing matters — doing the wrong thing first can worsen symptoms.
